The PSAP/EMS forms sent to PSAP/EMS agency can be found on the Location Details page under PSAP Notifications & EMS Notifications sections.
How do I navigate to find the PSAP/EMS form(s)?
On the Home Page, Click on the “Locations” tab.
All locations will be listed with the no. of AED units, Readiness Alerts, Exp. Alerts, and Exp. Warning. Click on the Location name to view the Location Details page.
On the Location Details page, scroll down to bottom and you will find the PSAP Notifications & EMS Notifications sections. Under PSAP Notifications, click on the form link and it will open up the form as PDF file which can be printed or saved on your system.
Similarly, Under EMS Notifications, click on the View link and it will open up the form as PDF file which can be printed or saved on your system.
If PSAP/EMS forms are not present, then please ensure that:
1. AED’s placement is updated to the exact location where the AED is placed.
2. Location Admin is set for each locatoin with email and Phone number.
